Daxxify has been generating a lot of questions at our clinic. Patients who have been happy with Botox for years want to know whether this newer neurotoxin is actually better, or just newer. It is a fair question, and the answer is not a simple yes or no.
The short answer: Daxxify lasts significantly longer than Botox for most patients. Whether that makes it better depends entirely on what you are looking for. What Is Daxxify and How Is It Different from Botox?
Both Daxxify and Botox are botulinum toxin type A products. They work through the same core mechanism: blocking the release of acetylcholine at the neuromuscular junction, temporarily preventing targeted facial muscles from contracting. When those muscles relax, the dynamic wrinkles they create smooth out. Every neurotoxin on our menu, including Botox Cosmetic, Dysport, and Daxxify, works this way.
What sets Daxxify apart is how it is stabilized. Traditional neurotoxins, including Botox, use human serum albumin as a protein stabilizer. Daxxify replaces this with a proprietary synthetic peptide called Peptide Exchange Technology, or PXT. This peptide binds more persistently to nerve terminals and extends the duration of the effect. The toxin itself is not more potent. It simply stays active at the injection site longer.
The clinical evidence for this difference is real. In the SAKURA phase 3 trial program, the largest randomized controlled study ever conducted for a neurotoxin, Daxxify demonstrated a median duration of 24 weeks for glabellar lines. Approximately half of patients maintained results at six months, and some reached nine months before needing a follow-up treatment.
How Long Does Daxxify Last Compared to Botox?
Botox typically lasts three to four months for most patients. Some patients with larger, stronger facial muscles, particularly in the forehead and jaw, find it wears off closer to ten to twelve weeks. Others with lighter treatment needs may see four months or slightly beyond.
Daxxify lasts six to nine months for most patients in practice. That is roughly twice the duration of Botox. For a patient who currently books Botox appointments three or four times a year, switching to Daxxify could mean just two visits annually to maintain the same result.

It is hours of driving, scheduling around work and family, and the ongoing mental overhead of staying on top of rebooking.
It is also worth noting that Daxxify's longer duration does not mean the result looks more frozen or unnatural. The degree of muscle relaxation and the cosmetic appearance of the result are comparable to Botox at equivalent doses. What changes is how long the result lasts before you notice the muscles beginning to move again.

Daxxify costs more per session than Botox. That is a fact, and it is worth addressing directly. But the per-session price is not the right number to compare. The right number is your annual total across all treatments.
A patient who pays for three or four Botox sessions a year and switches to two Daxxify sessions often finds the annual investment is comparable, and sometimes lower. The higher per-session cost is partially offset by needing fewer sessions.
This calculation varies significantly by patient depending on dose, treatment areas, and how their individual neurotoxin metabolism works. Some patients find Daxxify holds well to nine months. Is Daxxify Better Than Botox?

Daxxify tends to be the better choice if you:

- Find that Botox wears off faster than you would like
- Travel to get treatment and want to minimize the frequency of visits
- Are primarily treating the glabellar region, where Daxxify's clinical evidence is strongest
- Want a human albumin-free formulation
Botox tends to be the better choice if you:
- Are treating crow's feet, forehead lines, or other areas beyond the glabellar region, where Botox has broader FDA-approved indications
- Use Botox for therapeutic purposes such as hyperhidrosis or chronic migraine, where Botox has significantly more approved indications
- Are new to neurotoxins and prefer starting with the most extensively studied and familiar option
- Are enrolled in the Alle rewards program through Allergan and want to continue accumulating points on Botox and Juvederm products
- Want a shorter treatment cycle, for example if you are trying a new injection pattern and prefer to reassess sooner
Importantly, Daxxify and Botox are not mutually exclusive. Some patients use Daxxify for their frown lines, where the duration advantage is most relevant, while continuing Botox for crow's feet or other areas. Who Is the Right Candidate for Daxxify?
Daxxify is appropriate for most adults who are already good candidates for neurotoxin treatment in general. A few specific profiles that tend to respond particularly well:

- Patients who metabolize Botox quickly: if you consistently find that Botox wears off at ten or eleven weeks rather than fourteen to sixteen, Daxxify's longer-binding peptide mechanism may work better for your biology
- Patients focused primarily on glabellar lines: the eleven frown lines between the brows are where Daxxify's FDA approval sits and where its strongest clinical evidence applies

What About Dysport, Xeomin, and Jeuveau?

What to Expect When Switching from Botox to Daxxify
Transitioning to Daxxify is straightforward for most patients. A few practical things to know:
Timing
Schedule your first Daxxify treatment when your current Botox or Dysport has fully worn off. We do not recommend layering neurotoxins while one is still active, as it complicates dose assessment.
Onset
Initial smoothing typically appears within two to four days of your Daxxify injection, with full results visible at approximately two weeks. This is similar to or slightly faster than the typical Botox onset of three to seven days.
Units and dosing
Unit counts are not directly transferable between neurotoxin brands. Your provider will determine the appropriate Daxxify dose based on your anatomy and treatment areas, not a simple conversion from your previous Botox protocol.
Aftercare
Standard neurotoxin aftercare applies: avoid rubbing or massaging the treated area for 24 hours, and avoid strenuous exercise, saunas, and lying flat for four hours after treatment.

Is Daxxify FDA approved?
Yes. Daxxify received FDA approval in 2022 for the cosmetic treatment of moderate to severe glabellar lines in adults, and is also FDA approved for cervical dystonia.
